Structured Nutrition Education Sessions
Telephone Reassurance is a scheduled calling service provided on a regular basis to older persons in their homes to (1) help reduce isolation, and (2) monitor the older person’s safety, well-being and need for additional services.
The program provides transportation services to older adults residing in the program’s catchment area that enable older adults to enhance their mobility and participate in community life.
